Considering the importance of chlorine disinfection process of water supply systems, this study aimed to evaluate the conformity of residual chlorine and its implications for public health in the region of Agreste Pernambucano. The information was obtained from the National Sanitation Information System and the Brazilian Institute of Geography and Statistics. The methodology included normality verification using the Shapiro-Wilk test; hypothesis testing using the Student's t-test; and Pearson's correlation analysis. The results demonstrate that municipalities with non-compliant residual chlorine samples are more susceptible to acute diarrheal diseases. They also indicate the need to improve water treatment systems regarding the guarantee of minimum residual chlorine levels, especially in the Central Agreste region, where most of the non-compliant municipalities are located
