EFICÁCIA CLÍNICA DA CLOREXIDINA NA INATIVAÇÃO DAS METALOPROTEINASES DA MATRIZ: UMA REVISÃO INTEGRATIVA

Introdução: A degradação da camada híbrida pelas metaloproteinases tem sido apontada como um fator importante para a longevidade da interface dente-restauração. Em estudos laboratoriais a clorexidina tem atuado como agente inibidor dessas metaloproteinases, porém não existem evidências robustas sobre a eficácia clínica desta substância. Objetivo: Avaliar, através de uma revisão integrativa da literatura, a eficácia clínica da clorexidina na preservação da camada híbrida em restaurações diretas de resina composta. Metodologia: Realizaram-se buscas sistemáticas nas bases de dados PubMed, Scopus e Embase, incluindo apenas estudos primários in vivo e ex vivo, publicados em inglês, sem restrição temporal. Após a leitura de título e resumo dos 571 artigos identificados, 561 foram excluídos por serem revisões, estudos in vitro, em animais ou em outras áreas da Odontologia. Resultados: Foram selecionados 10 estudos para análise integral, todos no modelo boca dividida. Cinco estudos ex vivo examinaram restaurações classe I usando microscopia eletrônica de varredura e testes de microtração com resultados favoráveis ao uso da clorexidina em relação aos grupos controle. Já os estudos clínicos, analisaram restaurações classe V em lesões cervicais não-cariosas, usando os critérios United States Public Health Service e World Dental Federation com ausência de diferenças significativas entre os grupos experimentais e controle. Conclusão: Devido à escassez na literatura sobre evidências clínicas que sustentem a inclusão da clorexidina no protocolo adesivo, sua aplicabilidade ainda é questionada, o que aponta para a necessidade de novos estudos que indiquem evidências mais robustas e conclusivas.
